What you will need to take this test:
1. A well-lit area at least ten feet long.
2. Another person to assist you, if possible.
3. A pen or pencil to record the results.
4. A flashlight, if possible.
5. Tape or a tack to put the chart on the wall.
6. A 2 - 3 inch square piece of opaque paper.

How to take the test

1.	Sit exactly ten feet from the wall.
2.	After you have measured ten feet, print out the eye chart. Use tape or a tack to hang the chart on the wall. The chart should be hung at eye level when you are seated. The test should be taken in daylight or in a very well-lit room.
3.	Wear the glasses you use for distance vision. If you have bifocals, wear them.
4.	Cut a 2 - 3 inch square piece of opaque paper to use as an eye cover. Cover your left eye with this square.
5.	The person assisting you should now point to the letters on the chart one at a time with the flashlight, starting with the top line of letters.

Read the letters out loud. You should continue until you have either read all letters correctly or until the letters are too difficult to see clearly.

Recording the results
Next to each line of letters is a number. Find the number next to the last line you were able to read correctly in its entirety.

This is your score. Write down your score.

Go back and read the chart again, this time covering the right eye. Record the results for your left eye.

If you were able to read the 20/40 line with each eye, go to Test 3 and omit test 2. If you were not able to read the 20/40 line with one or both eyes, you should now take Test 2.
